Case presentation:
You are coming on to your evening shift. You have 2 post ops and 3 medical patients to care for.
You are on the list to receive the 2nd admission from the ED. One of you post ops is a 68 year old
woman. She has no medical problems. She has no known allergies. She is two days post-op
following hip fracture surgery. No problems were noted at nursing sign-out other than c/o pain,
for which she was receiving non-narcotic pain medication (Tylenol) with good relief.
When you perform your initial assessment on this patient, you find her to be confused.
When you call the resident physician on duty, how would you state your concerns and question?

Over the phone the resident physician asks that you obtain the following tests: CXR, ABG, EKG,
and routine blood work (chemistry studies and complete blood count), UA and urine C&S.
Forty minutes later, the resident has called back and reported to you that the CXR suggests
pneumonia, and the resident orders an IV antibiotic. Two hours later, as you enter the room to
start the first dose of the antibiotic, you note that the patient is more short of breath and more
confused. Her color is now gray.
You place an SPO2 on her and get VS. The patient’s O2 sat is now 88% on 50% face mask, and
her respiratory rate is 30/minute.
You are uncomfortable about keeping her on the general unit, and feel she needs more rigorous
monitoring and nursing care, and are worried about how you will care for your other 4 patients
and handle an admission. You ask if the resident if the patient should be moved to the ICU, but he
states he wants to first see how she responds to the antibiotic. You request that the resident reevaluate the patient.
