Chapter 6 of the Cummings, T.G., Worley, 2019 – Organization Development and Change, 11th Ed textbook provides a summary overview of the ways in which a change agent or organizational development consultant can collect data about an organization. Ideally, data collection is intended to help identify and clarify a problem that needs to be addressed for continued organizational performance. One method of collecting data is the very familiar survey. I would like you (The Writer) to design your own survey instrument. Think of a situation at work (NASA Johnson Space Center Mission Operations, or Engineering Division) or at school (The University of Houston) that you think needs to change and design a data collection survey that would help you understand the problem. The entire design is up to you. Your discussion should briefly describe the situation and why it needs to change. You should identify what populations should be sampled and you should provide the questions you will ask. The survey has to have at least 5 questions, but no more than 10. This exercise aims to get a feel for how difficult it is to design a survey instrument that can uncover useful information about an organizational problem.
