Project 2: Quality Analysis of a Nursing Sensitive Outcome The purpose of this project is to give students the opportunity to perform a simulated quality analysis. Students will choose an outcome problem and either a structure or process variable that might improve the quality problem in a simulated quality improvement initiative. For this project, the student will assume that the fictitious Capital Hospital (CH) has received a quality performance report. In response, CH conducted a quality improvement plan by implementing a change in either a selected structural variable or evidence-based process variable to improve the patient outcome problem. For the project, students will analyze simulated data on the two variables, will report on the results and make recommendations for continuous quality improvement. A dataset called the Capital Hospital dataset is provided on the course management for this project. i. Identify a patient outcome/healthcare problem from the CH dataset and a potentially related structure or process variable that may improve the outcome. Students are encouraged to use the dataset provided by the instructor (The Capital Hospital Dataset). Students may use data from a professional practice setting; however, students must obtain written permission to use data from a professional practice setting and must assure patient privacy and confidentiality. If students do not have data from a professional practice setting, they should utilize the data set provided by the instructor. From the dataset, identify and select one outcome and one structure or process variable that might improve the problem in a simulated quality improvement initiative. Organize and enter the data for the outcome and either a structure or process variable into a spreadsheet. Present the data set in a table that displays the name of the variables/metrics (displayed in rows) and the dates/times (displayed in columns). The table should include data from 2 variables: 1 variable concerning the identified nursing-sensitive patient outcome and 1 variable concerning the identified structural or process factor that may be related to quality improvement concerning the nursing-sensitive outcome variable. ii. Analyze the data you entered into the spreadsheet by graphing the 2 variables on a double axis graph to display trends of the outcome and the structure or process variable. A double “Y” axis graph is required when the variables have been measured on two different scales of measurement. If the scales of measurement are the same for both variables a single “Y” axis graph should be chosen to display the trends. iii. Perform an online library database search through the Capital Library website for a minimum of 5 professional healthcare journal articles concerning the background, significance and evidence-based management of the clinical problem. Together these articles will be sued to develop the background, analysis, interpretation, and recommendations in a quality analysis report (paper) described below. The guidelines for this analysis and report are given below. www.qualitynet.org AHRQ National BenchMark Tables CDC Healthcare Associated Infection Data Quality Analysis Paper Guidelines / Rubric: For this paper, consider that the data presented in your table and graph concerns the analysis and results from a study of a simulated quality improvement initiative from the fictitious Capital Hospital. Assume that Capital Hospital has implemented the structure or process variable that you selected as a plan to improve the patient outcome as a part of their ongoing quality improvement process. The spreadsheet analysis (table and graph) is an analysis of the current state of the identified problem in relation to the structure or process variable that was implemented. This is to be written as an internal report to CH about the problem and the analysis. The report should include background on the problem, spreadsheet table, graph, interpretation including comparison of the outcome performance against a national benchmark, and recommendations for continued quality management/improvement concerning the problem. A. Introduction (5 points): State the purpose and organizing plan of the project/paper. Identify the quality problem/outcome that will be analyzed from the chosen data set and the structure or process that was implemented to improve the outcome. Preview the intended plan and intended outcome of the analysis in relation to the identified problem. The thesis statement should also draw the connection between nursing-sensitive outcomes, the use of informatics to transform data to information and the use of evidence in a process of overall quality improvement. B. Background (10 points): Discuss the background and significance of the nursing-sensitive clinical problem that you selected. Using at least three references from the literature, describe the nature of the problem, the incidence or prevalence of the problem, and the significance of the problem in relation to both patient outcomes, complications and healthcare costs. Describe structure and/or process variables that have been identified to improve the patient outcome from the literature. Describe best practices or care bundles from evidence-based research or standards that are recommended for the management or prevention of the problem. Identify a national benchmark or statistic for later comparison to the analysis in this project. (This discussion will serve as the basis for comparison or benchmarking for your quality analysis concerning the status of this problem evident in the data that you analyze. This 22 will also provide a basis to refer back to for recommendations for continuous quality improvement.) C. Quality Improvement Plan (10 points): Describe the plan/structure or process intervention that was implemented for the quality improvement initiative. Describe the best practices or care bundle or intervention that was implemented in detail. Describe a process for staff training on the quality improvement initiative using either a Learning Theory or Technology Adoption Theory as relevant. D. Method (15 points): Describe the method of qualitative analysis. Discuss the rationale for how a spreadsheet analysis would support the transformation of data to information that would lend insight into the nature of the problem and/or potential solutions to the problem leading to quality improvement in patient outcomes (Apply the Data to Wisdom Model in this discussion). Describe the data source that the data set was derived from. Describe the specific data set that you entered into a spreadsheet. Include information concerning the type of data, size of data set entered. Classify the variables under study according to the Quality Model (Identify whether the variables are structures, processes or outcomes). Display the data in tables with relevant charts or graphs from your analysis of the variables. Provide a rationale for the type of graphing / analysis used. (Include the table of the raw data from the dataset and the graph of the trend in the data. Present the tables and figures according to APA format in the paper.) E. Analysis / Interpretation (15 points): Discuss the interpretation of the transformation of data to information. Discuss the conclusions that can be made from the analysis (Table/Graph) in relation to the influence of the structure or process variable on the outcome variable. Compare the results of the data transformation to the literature that you reviewed for the project/paper using available outcomes in the literature as benchmarks as relevant. Apply a quality model in this discussion. Note: the results of the analysis may or may not have improved the outcome problem in the expected way depending on whether the structure or process variable was actually the cause of the quality problem at CH. Order and label all tables & graphs in APA format. Make sure that each table/graph is discussed and referenced in the paper. F. Discussion (10 points): Describe whether the change in the outcome variable is attributable to the structure or process intervention. Describe whether the status of the outcome variable in the analysis is consistent or inconsistent with the benchmark of comparison. Going back to the literature review, discuss other structures or processes that have been identified to influence the outcome 23 variable. Discuss whether other structures or processes need to be considered in view of this analysis. Discuss any strengths or limitations of this qualitative analysis. G. Recommendations (10 points): Make recommendations for continued quality improvement related to the identified problem. These recommendations should relate to the analysis of how the data in your analysis compared to the benchmark evident in previous discussion concerning the clinical problem. Recommendations should also include relevant other structure or process factors that should be studied in the in view of the results of this analysis and that were evidenced in the literature review. Apply a quality improvement process model in this discussion. The recommendations should address: What should Capital Hospital do next in the quality Improvement process. The recommendations should also present a strategy to address change management or learning theory in this process to assure either stability of the quality improvement or continuous quality improvement. Make sure to consider change theory i.e Lewin, Rogers, etc. H. Conclusion (5 points). Briefly summarize the results of this analysis. Describe how evidence-based practice and informatics worked together in the quality improvement process to improve the nursing-sensitive outcome in this quality improvement initiative. I. References (10 points): Adequate text referencing is required according to APA guidelines and standards. Include a complete reference list that contains all articles/resources used for the project and are cited in the text. The paper and the reference list must conform to APA format. A minimum of 5 references are required. All references in the text should also be listed in the reference list. All references should be listed using the primary author. If a secondary source is utilized, APA guidelines for citing secondary references must be followed. J. Style (10 points): Writing style, grammar, and syntax must conform to APA style. The paper must be professionally presented and exhibit organization, accuracy, thoroughness and critical thinking. Headings are required for each subsection of the paper. Follow APA guidelines for levels of headings. Deliverable: Submit the completed paper with reference list, tables & graphs to the instructor on the date due. Papers should be submitted to the course management system dropbox folder for this assignment.
