Lesson 4 Discussion
An adult patient with a chronic myelogenous leukemia sits down with you to discuss his questions and concerns about his upcoming bone marrow transplant. He has already received some educational materials and participated in a family conference during which health team members described the procedure and potential complications. He has been told that he has a risk of graft rejection or graft versus host disease (GVHD), but he does not understand the distinction (Chapter 12, Learning Objectives 1, 2, 10, 11).
What are the similarities between graft versus host disease and graft rejection?
What are the pathophysiologic differences between graft versus host disease and graft rejection?
How would these differences be manifested clinically?
Studies have shown a protective effect of mild to moderate GVHD in cancer patients who have had a bone marrow transplant. Based on your understanding, can you explain these findings?

Chapter 18 Discussion
Ed Beatty is a 52-year-old client with a history of chronic renal insufficiency who presents to the medical clinic reporting a severe sore throat. He is diagnosed with a streptococcal pharyngitis. He is informed by the primary care provider that he will be treated with an IM injection of penicillin G. (Learning Objectives 2, 3, and 9)
1. What information should be collected from Mr. Beatty before the administration of the penicillin G?
2. Mr. Beatty asks why he cannot receive the medication in oral form. How should the nurse respond?
3. Describe the equipment that the nurse should have available before administering the medication to Mr. Beatty. Why is this necessary?
4. After receiving the penicillin G, Mr. Beatty experiences cardiac arrest. He is successfully resuscitated. Discuss the potential causes of the cardiac arrest.
5. Mr. Beatty has recovered from the arrest and the nurse is preparing his discharge teaching. What information related to the penicillin G should the nurse include?

Chapter 1 Discussion
You are preparing to discharge a 43-year-old male patient who has received treatment for a fracture of the right tibia. You provide the patient with a prescription for a narcotic analgesic to be used as needed for pain, along with information about the medication. The patient states, “I don’t need the prescription. My cousin just had surgery and didn’t take all of his pain medicine. I can just use his. That will be cheaper for me, anyway, since I don’t have insurance.” (Learning Objectives 3, 4, and 5)
1. a. Discuss the legal and therapeutic implications related to the patient taking narcotic analgesics that have been prescribed for another person.
b. With these implications in mind, how should you respond to the patient?
2. How should you respond to the patient’s concern about the cost of his medication?
Instructions:
Your discussion post should be at least 140 words and include at least one 100-word response to a classmate. Follow APA guidelines for references. At least one reference is required to support your response.

The first step of the evidence-based practice process is to evaluate a nursing practice environment to identify a nursing problem in the clinical area. When a nursing problem is discovered, the nurse researcher develops a clinical guiding question to address that nursing practice problem.
For this discussion question, you will create a clinical guiding question known as a PICOT question. The PICOT question must be relevant to a nursing practice problem (coordination of health care, assessment, education, patient support, trauma prevention, recovery, health screenings, etc.).
After reviewing the relevant topic materials:
Select a nursing practice problem of interest.
Develop a PICOT question using the template and example provided below.
Identify two articles, one qualitative and one quantitative, related to the problem of interest. Provide an APA-formatted reference for each article, and explain how each relates to your PICOT question. Be sure to identify which article is qualitative and which article is quantitative. These articles may be used in this week’s assignment.
PICOT question intervention format:
In_______________(Population), how does _______________ (Intervention) compared to _______________ (Comparison) affect _______________ (Outcome) within______________(Time, optional)?
Example: In critically ill patients in an Intensive Care Unit (ICU) (P), how do daily 2% chlorhexidine cloth baths (I) compared to daily disposable non-antimicrobial cloth baths (C) affect the incidence of health care-associated infections (HAIs) (O)?
Should be a minimum of 200 words and include at least two references cited using APA format.

In weeks 5-7, you prepare for your course Signature Assignment (due in week 8) by composing annotations from research articles and sharing your research with the class. Review the Signature Assignment directions in week 8 to make sure your research this week meets the requirements.
This assignment has three steps, the second of which MUST be completed
Research a condition caused by pulmonary disease or injury and find two peer-reviewed sources.
Record a brief summary of your annotations in VoiceThread
Step 1: Research a condition caused by pulmonary disease or injury
Select a condition caused by pulmonary disease or injury (such as hypercapnia, hypoxemia, or acute respiratory failure) from this week’s content. Using the Gangwish Library or another peer-reviewed scholarly source, locate two scholarly peer-reviewed articles relating to the chosen disorder.
In a Word document or a PowerPoint slide, write an annotation for both articles. Format your document with your name and your selected disease at the top. In the body of your document, type the APA-style reference. Refer to the Writing the Annotated Bibliography OU LibGuide for examples of annotations.
You do not need a title page or a reference page. Use 12-point font, single-spacing and try to fit both annotations on one page/slide.
Step 2: Present your research to our community of learners
Use a video discussion tool called VoiceThread (VT) to present your reasoning (i.e., argument related to your chosen disease for this week) to your peers in a video recording. Your presentation should provide a brief summary of the articles and explain how the research you have gathered informs:
the care management of the patient,
the promotion of the patient’s health, and
the prevention of further disease.
Your recording should be no more than 8 minutes in length. Include the following:
Add your annotations document to the Pulmonary Disorders Research Annotations VT.
A video or audio recording sharing a brief summary of each article and your reasoning for selecting the articles.
Posting in VoiceThread:
Post your annotations and video or audio presentation in VoiceThread
Click the “Week 5 Pulmonary Disorders Research Annotations” link to launch VT.
Click “Launch” to open VoiceThread in a new browser tab.
Click the “Start Assignment” button to begin.
Add your annotation document by uploading the file from your computer (or use one of the other supported options).
Edit the Presentation name to be descriiptive of your topic.
Add a video or audio comment to record your rationale.
Click the “Submit” button to submit your VT to the “Student Gallery.” This will close VoiceThread.
Step 3: Respond to your peers
Collaborate and have a conversation with your peers about how the research they shared relates to your own clinical reasoning and knowledge. Reviewing the annotations of your peers can also help you find research for your Signature Assignment.
Go back into the VoiceThread assignment to click on the “Student Gallery” link to view the presentations of your peers.
Reply to at least two peers’ presentations to further the discussion (and findings) as it relates to your own clinical reasoning and knowledge.
Submit your annotations and video or audio presentation in VoiceThread

Review the Case Example on page 260 in Chapter 12 in your text. Using the skills you have already gained from previous courses, what would you say to the nurse who told Mrs. L, “I am sure that delaying your lunch until we get you settled won’t hurt you a bit?” How can the nurse who made these statements improve her biases: “Isn’t it sad that all she can seem to focus on is her lunch? She needs to be on a strict diet.”

Clinical Documentation: Complete History and Physical Examination (H&P) Guidelines
The purpose of this assignment is to (1) assist the student in developing the skill of interviewing patients to obtain a complete history and perform a correct physical exam and (2) clearly and succinctly documenting the pertinent data in a logical, organized format in the patient health record, using appropriate nursing and medical terminology. The student will gain experience in formulating a problem list.
Directions:
Select a patient seen in your clinical setting on whom to gather a history and perform a physical exam. Document the history and the physical exam in a standard, organized format using the guidelines below. A sample comprehensive history and physical examination may be found on pages 33-35 of Sullivan (2019). Save the assignment as
LastName.FirstName_NURS7710_H&P_1. The second submission should be labeled with the number two (2).
Documentation should:
• have no errors in sentence structure, word usage, grammar, or punctuation
• be clear, well-organized and logically progressive, and
• be succinct with no unnecessary verbiage.
Correct medical terminology and approved abbreviations should be used.

Including all of the above components of the comprehensive health history add the following:
COMPLETE PHYSICAL EXAMINATION (PE)
1. Document physical exam in the following order using a systems-heading format, not narrative format:
a. General assessment
b. Vital signs: temperature, pulse, respirations, blood pressure, height weight, body mass index (BMI)
c. Skin
d. HEENT
e. Neck
f. Thorax and Lungs
g. Breasts and Axilla
h. Cardiovascular
i. Abdomen
j. Lower Extremities
k. Musculoskeletal
l. Nervous System
m. Genitourinary or gynecological and rectal
2. Do NOT document “WNL” or within normal limits for your physical exam findings. You must describe your actual exam findings using appropriate medical terminology. Document using measurable terms.
LABORATORY OR DIAGNOSTIC STUDIES
1. Tests and results (i.e., CBC, Rapid Strep-Test, Blood Glucose, Hemoglobin, Urine Pregnancy Test, Urinalysis, etc.) Not what you ordered and is pending, but the results of the tests completed.
PROBLEM LIST/ASSESSMENT
1. After you have obtained the history and physical exam of your patient, construct a problem list that should include all active or pertinent medical problems, including those discovered on ROS, physical exam, or labs/studies.
2. List at least 3 differential diagnoses. Include rationale for ruling in or ruling out diagnoses.
3. State each final diagnosis. There may only be one.
4. If diagnosis is unknown at this time, you may use the symptom (i.e., headache, fever, shoulder pain).
5. Do not write “Rule Out ______”
6. Include a brief statement (your rationale) supporting each diagnosis. This rationale should be based on subjective and objective pertinent positive and negative findings.
7. No plan is expected in this course
