I had a 2 week placement at an age care at a student nurse and could use this as my reflective blog for a nursing journal for others students going on placement at an aged care and needing to understand standard 1)

Social Change is at the core of Walden’s mission as a university. The sociological definition of social change is the “transformation of culture, behavior, social institutions, and social structure over time (University of Minnesota, 2016).” However, Walden University’s mission of social change is even more succinct. It is “people working to improve the lives of others (Walden University, n.d.).” As a DNP-prepared nurse, examples of the kinds of positive social change you might advocate for include implementation of policy that decreases infection rates in a vulnerable population, organizational change to provide immunizations for older adults with limited access to insurance, or respite activities for the staff during work hours.
As an advanced practice nurse, you are and will continue to serve as an agent of change in all you do. Think about the positive impacts you have on patient care, the organization for which you work, the communities you serve, and even the field of nursing in the largest sense. Consider the ways in which earning the DNP degree will support you in your advocacy for positive social change and your commitment to fostering innovation for change in nursing practice.

Begin to research and gather information about part two of the project: Creating an electronic media message. Review the list of app building and social networking tools included within the Reading and Resource section of this module.
Include within the outline 3 to 4 sentences describing:
• The issue your media message will address and why you chose it.
• What outcome that you intend to achieve with the message.
• Who will benefit from the information (targeted audience).
• An overview of the message you will be crafting.
• Which media tool will be used and how it will be formatted.
• Potential strategies you will use to achieve the expected outcome.
• How privacy or security issues of concern will be mitigated.
Use the APA 7th edition to format the paper in a Word document. Use at least 3 relevant, contemporary, and scholarly references. Synthesize the information gleaned using reference citations as appropriate. Avoid using the words of others as direct quotes. Add section headers in the paper to assure all the required components of the assignment are included.

The social determinates of health include factors other than access to health care, such as socioeconomic status, economic stability, access to quality education, physical environment, support networks, employment, etc. Although efforts to improve healthcare within the United States usually point to responsibilities within the healthcare system, there is a growing recognition that these social determinants of health are also critical—from the individual to the population level.
As a DNP-prepared nurse, you will consider and address the social determinants of health when working with patients. Think about the ways in which this will positively impact your patients’ experiences and outcomes, as well as the communities you serve.
Review the Learning Resources that address the importance and value of taking the SDoH into consideration as a DNP-prepared nurse.
Consider why and how you intend to begin shifting your thinking and practice related to the SDoH from the relatively smaller scope of patient care to the broader perspective of organizations, communities, and the larger field of nursing (e.g., advocating at the policy level, meeting with stakeholders, utilizing esearch and data, etc.).
Select one SDoH Domain that is of particular interest to you on which to focus for this Discussion.
Using Walden Library and the internet, search for and identify three scholarly, peer-reviewed articles (within the last 5 years) that address the SDoH Domain you selected from the broader perspective of organizations, communities, and the larger field of nursing.

In this two-part deliverable assessment, first, create a 5–7 slide elevator-speech style presentation that you will present to stakeholders that provides an overview of policy implementation, including rationale and data that reflect the need for the new policy. Second, create a detailed, annotated training agenda for the pilot team that will accompany your presentation.
In this assessment, inform the leadership stakeholders of a planned pilot project to test the implementation of your new policy.
