Outpatient care is described as an individual (patient) who receives healthcare services (such as surgery) on an outpatient basis, meaning they do not stay overnight in a hospital or inpatient facility. Many insurance companies have identified a list of tests and procedures (including surgery) that will not be covered (paid for) unless they are performed on an outpatient basis. The term outpatient is also used synonymously with ambulatory to describe healthcare facilities where procedures are performed (Anderson, 2022).
Reimbursement, technology, utilization control, and social factors are key changes that contribute to the shifting of balance between inpatient and outpatient services. With the changes being implemented, and incentives for reimbursement of outpatient care, hospitals have created outpatient services to help with the decline in income from inpatients. The development of new diagnostic and treatment procedures and less invasive surgical methods has enabled services in outpatient settings that previously required inpatient hospital stays (Shi & Singh, 2019). Payers are trying to minimize hospital stays so closely monitoring patients while in the hospital is increased to try to minimize hospital stays. Patients generally prefer receiving health care in home and community-based settings. Indeed, most people do not want to be institutionalized unless absolutely necessary (Shi & Singh, 2019).
Continuity of care is a defining attribute of primary care and a core element of the Institute of Medicine’s definition of primary care. Continuity is generally recognized to have three dimensions: continuity in information, in management, and in the physician-patient relationship. Relationship continuity is the ongoing interaction of a patient with one physician, which results in increased knowledge of patient preferences, better communication, and improved trust (Sharma et al., 2009). Outpatient healthcare delivery has been shaped in many ways but one of the main foundations is primary care. Services beyond primary health care have become an integral part of outpatient services. As specialist services increase in supply and use, specialist physicians have become more integrated into the primary care system; however, improvement is still needed to ensure seamless coordination of care between specialists and primary care providers (Shi & Singh, 2019). Primary care can be described as basic or routine care. Primary care should be the point of entry into a health services system (Starfield, 1992, p. vii), that is, the first contact that a patient makes with the health care delivery system (Shi & Singh, 2019). One of the main functions of primary care is to orchestrate health care services between the patient and the health care system. Coordination of an individual’s total health care needs ensures continuity and comprehensiveness. These goals of primary care are best achieved when the patient and the provider have established a close relationship over time (Shi & Singh, 2019).
Coordination of an individual’s total healthcare needs ensures continuity and comprehensiveness. These goals of primary care are best achieved when the patient and the provider have established a close relationship over time (Shi & Singh, 2019). Primary care is considered as essential care and in an ideal healthcare delivery environment primary care is the base.
Outpatient services are important in hospital settings and help with revenue and should be utilized. Outpatient services now transcend basic and routine primary care services, as many general medical and surgical interventions are provided in ambulatory care settings. In response to changing economic incentives in the health care delivery system, numerous types of ambulatory services have emerged, and a variety of settings for the delivery of services have developed. In most settings, patients visit delivery sites to receive services. In other cases, services are brought to the patients (Shi & Singh, 2019). Administrators need to consider how important outpatient services are when understanding why it is an important element of their business strategy, outpatient services are necessary for effective patient-centered care. Philippians 2:4 reads. “Not looking to your own interests but each of you to the interests of the others.” The care of the patients should be the main priority.

Health Care Systems – Outpatient Care
Outpatient care is a healthcare consultation, treatment, procedure, or other service administered without an overnight stay at a hospital or medical facility (Shi & Singh, 2023). Outpatient care can be administered in various facilities, such as primary clinics, urgent care clinics, ambulatory surgical centers, and community health centers. You can receive outpatient care through colonoscopies, physical therapy, mammograms, diagnostic imaging, and lab work (Shi & Singh, 2023).
The fundamental changes that have been instrumental in shifting the balance between inpatient and outpatient services can be classified under the broad terms of reimbursement, technology, utilization control, and social factors (Shi & Singh, 2023). Reimbursement plays a factor in the switching of balance between inpatient and outpatient services because outpatient care costs less than inpatient care (Shi & Singh, 2023). With the cost of outpatient care being…. in the interest of both the healthcare system and the patient, utilizing these services will lower the cost of healthcare overall. A study compared the cost of Managing complex type 2 diabetes patients at a primary care clinic and a hospital outpatient department. The results showed that the yearly savings for a single patient to receive diabetes management at a primary clinic was $365 and 93.7 percent of the test subjects saw a cost reduction (Donald M, 2021). Technological factors contribute to the shift of balance between inpatient outpatient services with the development of new diagnostic and treatment procedures because they are less invasive and enable the services to be formed in an outpatient location so the patient can go home later that day instead of staying in a hospital (Shi & Singh, 2023).
Outpatient care supports the continuity of care in delivering health services by providing services that historically were only found in hospitals outside of hospitals at a lower cost. Some examples of this are hospital outpatient clinics, mobile diagnostic imaging services, telephone or internet triage, hospice care, and various other services. Having these services in the outpatient setting will cut the cost of healthcare for the healthcare system and the patient. With this cost reduction, the patient has more money to live their life and be a member of society, and the healthcare system can have more funds to run and improve the hospital system where needed. During the health pandemic, telehealth became an essential tool to provide care to those affected by COVID-19. Telehealth can be an approach that can be an effective and affordable healthcare system that aims to facilitate access to care, maintain the quality and safety of care, and engage patients, health professionals, and users of Health Services (Bouabida K, 2022). Healthcare systems have expressed their incredible will to maintain and promote the utilization of telehealth services to ensure patients receive the best patient care possible (Bouabida K, 2022).
Hospital administrators are starting to regard outpatient care as an essential key component of their overall business strategy due to a few factors. One of the main factors is that over the past 20 years, we have seen a decline in the hospital occupancy rates, which means there are empty beds in the hospital, which means lost revenue. With the growth of non-hospital-based outpatient services, there has been competition between hospitals and community-based providers. Healthcare system administrators must be competitive and focus on outpatient care to keep patients and revenue.
Luke 10:34 says, “He went to him and bound up his wounds, pouring on oil and wine. Then he set him on his own animal and brought him to an inn and took care of him.” (New International Version Bible, 2011, Luke 10:34). This to me shows a biblical reference of a doctor and how they were with the patient, treated the patient, and ensured that they made it home safely all while being under their care. This is what health care is all about, providing care for a patient and ensuring the best financial and physical outcome in this economy.

According to Buchbinder & Thompson, there are 5 key themes identified by practicing healthcare managers. These themes are interest in the profession and satisfaction, role of mentors, variation in preparation, and challenges faced by healthcare managers (2010). The themes mentioned in the text have helped and improved healthcare managers in their line of work. Most gave a background of what got them interested in this field of work. These themes are very personal, and some can relate. Thinking about themes of healthcare, I think of what’s the point of healthcare? With this question everyone should have an answer whether you’re in healthcare or not. The point of healthcare is to provide health care to patients in need of special care. What do you think is the point of healthcare?
When I think of the interest in the profession and satisfaction, it explains itself. You have interest in the profession, so you went for the job. Other hand, you get satisfaction when accomplishing something in the workplace. Examples of satisfaction through the workplace include “working closely with others who share a commitment to the helping profession, being challenged and never being bored in their roles, and having the chance to demonstrate their understanding and key skills in completing tasks and leading others (Buchbinder & Thompson, 2010). Everyone who loves doing their job should have satisfaction somewhere especially if good things are the outcome.
In the role of mentor, anyone can be your mentor. Someone who is not in healthcare can be your mentor. Someone who is younger than you can be your mentor. Proverbs 22:6 says Train up a child in the way he should go; even when he is old he will not depart from it (2001). God is saying train or mentor someone in the direction of success and as they get older, they will still be in that field because you mentored and help them on a success journey. Also, could be saying mentor them until retirement. That speaks a lot of words for steering someone in the direction of success. “Mentorship, however formal, builds confidence and character, challenges our comfort zones, and creates lasting connections. And while mentorship is valuable in all walks of life, it’s particularly beneficial in the healthcare and medical professions due to their demanding academics, training, and job responsibilities” (Zansler, 2020).
Variation in preparation is all about the variety of ways you have learned to get into the healthcare management position. Some may be personal, the other may be through school. Either way, that learning technique has granted you interest in studying healthcare. You may need plenty of experience in healthcare and you should line up your KSA’s with the position that best fits you and take the steps that need to be taken to fill that role.
Challenges faced by healthcare managers can be hard. “Young and mid-career managers wrestle with the challenge of balancing family and career, a pressure felt particularly by many female managers” (Buchbinder & Thompson, 2020). Females have a hard time when dealing with children and babysitters versus men. That can be a challenge when you have a little one so young. Challenges can occur when someone is in school and managing a family also because you will get easily distracted. Healthcare management is not the only position where challenges can occur because your lifestyle can challenge you in any way possible. It is up to you to balance it and not let it overwhelm you. Psalm 86:7 says when I am in distress, I call to you, because you answer me (2001). Call on God for your troubles and he will answer you for what is best for you.
There are different growth and changes during healthcare management. Changes can result in more people and less hours worked. Can be less people and more hours worked. Changes can also be different workloads for someone than before. “Change management in healthcare is the process of implementing innovation, new perspectives, processes, and approaches to enable improvements in areas such as care delivery, patient utilization, satisfaction, and engagement (AKASA, 2023). I look at changes as being a good thing for the job, but it also can be a bad thing. I believe these changes will impact the healthcare system successfully because as time goes by, there will need to be time for change. A change in the workplace can happen daily, weekly, or even monthly. New change causes for new responsibility and anytime the workplace has implemented change, it is a new approach to something. New changes in technology can support the clients in a better way and make it easier for healthcare workers to operate. The impacts on both care settings should be around the same because all changes should be used to elevate a company and not tear it down.
“Employment of medical and health services managers is projected to grow 28 percent from 2022 to 2032, much faster than the average for all occupations” (U.S. Bureau of Labor Logistics, 2023). As far as employment for managers, the growth will impact the organization in a good way because the more workers the better. Even if there is no available work for them now as the years go on there will be job opportunities and more work for managers that should be filled. In any healthcare position, as years go on, the position percentage will grow in 5 to 10 years.

Key Themes:
Interest in the Profession and Satisfaction:
“Managers have high interest in the healthcare profession and derive significant satisfaction from their roles.” (Buchbinder & Thompson, Pg. 196, 2010) Most healthcare managers were once in the clinical role, as medical assistants, nurses, doctors, or something else. Part of what makes them successful managers is that they’ve experienced things from a different perspective within the clinical setting, so they are familiar with how things work, and can often give better ideas on how things should be run. “Job satisfaction is indispensable in the daily life of the workforce, and the mechanism that drives job satisfaction requires the attention of the management of corporate organizations.” (Freitag, et al., 2022)
Role of Mentors:
“Mentors are role models who also act as guides for students’ personal and professional development over time.” (Henry-Noel, et al., 2019) Typically, each healthcare manager has someone who has helped them succeed in their position. This could be anything from suggesting they would be good at a healthcare manager position, training them for the position once it has been obtained, sometimes even being there as a second opinion or someone to bounce ideas off of once they have already successfully become a healthcare manager. Mentors are important because they are there for you to fall back on when you need additional assistance or expertise in situations. Mentors can be there for professional help and can also be there for people when they seek personal assistance. Having someone to talk to, when you are unsure, or stressed, is a good way to manage your thoughts and come out on the other end with results.
Variation in Preparation, Experiences, and Roles:
“There are different paths to the type of career in healthcare management that you may desire.” (Buchbinder & Thompson, Pg. 198, 2010) Some people such as I, start by volunteering in a healthcare facility, or running EMS on an ambulance. Volunteering is helpful to the staff at the facility, and beneficial in helping you to decide if a career in healthcare is something you want to pursue. As a volunteer you can see the different areas of the facility, the different jobs that people hold, and what those jobs entail. Sometimes, seeing what goes on in a facility as an outsider is enough to help you recognize what needs to be done to better the facility, and the push some people need to pursue healthcare management so that they can make a difference for the future.
Challenges Faced by Healthcare Managers:
There are, unfortunately, several challenges that healthcare managers must face in their daily lives. Managers must oversee other employees, and make sure they are doing their jobs effectively and efficiently. In doing so, it is also their responsibility to make sure they are getting their hours that they are scheduled for. Managing productivity is a large part of being a healthcare manager. You must make sure you have enough staff each day to safely take care of patients, but also not too many people on the clock to where it becomes counter-productive to the checks and balances portion of being a manager. You must make sure that your staff are happy, otherwise you must deal with staff turnover, and try to hire qualified personnel to replace the ones you’ve lost. Healthcare managers must deal with insurance companies, both on the staff side and the patient side. They also must deal with the rules and regulations of the hospital. They must make sure they follow appropriate steps to ensure quality, but also safety with every procedure and treatment that is provided. “Safety standards and compliance regulations are always changing in the healthcare industry and it’s important to stay on top of the ever-changing regulatory landscape.” (Brenner, 2021)
Growth and Changes
Long Term Care, Elder Care, and Geriatrics:
With the baby boomers getting to the age where they may no longer be able to care for themselves, there will be more people looking for care and assistance. It has been suggested that healthcare managers not only be part of hospital settings and outpatient doctors’ offices, but also become part of facilities such as hospice, assisted living facilities and nursing homes. Having someone with experience and expertise would likely be helpful in ensuring the best care possible for those who are seeking it.
Information Technology and Data Management:
As healthcare facilities begin to lean more towards the use of technology in their care, there will be “greater opportunities for managers with detailed knowledge, understanding and skills in managing data.” (Buchbinder & Thompson, Pg 201, 2010) It is helpful to have someone within your practice or facility that is skilled in the use of technology, as not all of us are. This person can help with system updates, issues with the technology being used, among other things.
Medicare, Medicaid, and Other Federal Health Programs:
As more people are requiring the help of these federally funded insurance programs, it is important to not only understand each of them and how they work, but also understand how these services will either hurt or benefit you and your practice by accepting or not accepting them.
Performance Improvement/Quality/Patient Safety :
Some healthcare managers have more experience in the customer service aspect of patient care than others. This is beneficial because those are the people who can help figure out what areas need the most improvement, and how to better the quality outcomes of patient care and safety. Patient safety is at the top of the list of importance when it comes to taking care of them, so it’s necessary to take all the right steps in ensuring it is done successfully, and in the most efficient way possible.
Clinical Leadership:
As mentioned before, and suggested by one of managers during their interview, the best leaders often come from the bottom. The best leaders have started at the bottom of the totem pole and worked their way up into the management position. They have worked in each part, or at least some parts of the facilities where they are now managing and know what goes on, and what goes into each position. This helps them understand their employees better because they can honestly say they have been there and understand what needs to be done to improve things.
The impact of these changes will help facilities grow and become more patient and employee focused. Having management who has been where you are now, and experienced the things you experience will help them make decisions that are best not only for the practice but also for you as an employee. It will help patients be treated at a lower cost and provide more options for treatment. Proverbs 3:27 says “Do not withhold good from those to whom it is due, when it is in your power to do it.” (What Does the Bible Say About Caring For Others? (2019) Becoming a healthcare manager means that you want to do good by your patients and employees. It means you care, and you want to make a difference so that things will be better for the future.

1. I have not known anyone that I know of who has suffered elder abuse or a crime against them as an elder. I have known an elder who abused drugs and alcohol and ended up dying alone and not being discovered for days. Being older, crankier, and an addict tends to push people away. In this case, many people tried to help over the years until it just became impossible. Resources or not, I do not know that anyone could have done anything differently other than the addict himself.
I used to do medical billing and I would have to call people for their past-due bills. Some of them would tell me they got $800.00 a month from social security and they did not even know how they were going to eat for the rest of the month let alone pay a medical bill. I think it is a crime to garnish these people’s bank accounts who can barely pay rent and eat. If it was not for them, we would not be here. We need to honor our elders and take better care of them as a society. We literally pay into social security our entire working lives, with the idea that we are investing in our future well-being and those who are currently receiving social security however, it is not enough alone as many elders are not doing that well.
A lack of money is not the only problem, some people are on social security and have other assets but they have caretakers who take advantage of them. I believe we need to make sure they are being protected financially.
“Financial or Property Exploitation means illegal or improper use of an elderly or adult with a disability’s money, property, or other resources for monetary or personal benefit, profit or gain. This includes, but is not limited to, theft, misappropriation, concealment, misuse or fraudulent deprivation of money or property belonging to the elderly or adult with a disability” (Civil Financial Exploitation, (2022).
How do you think we can protect our elder’s assets and prevent them from being taken advantage of by caretakers but also acknowledge that it costs money to take care of them?
2. There once was a lady that was known to some people in my neighborhood that had care giver come to her house daily and mistreat the women. this women would do things to this lady because she figured that she didnt have family close by that they could do what ever to the lady. the lady was all alone so she tolerated anything the caregiver did to her always. how things get handled in my neighborhood isnt correct but that care giver got her comeuppins and never looked back to work with that lady again. The proper way to handle the situation would of been to call the cops as elder abuse is considered a felony. edler abuse can be physical, psychological, sexual , social, or financial. to report this type of abuse in nyc you can call 311 and they will get you to either the nys adult protective service line to take the proper steps to get the person under arrest.

1. In staten island senors who live in Nycha has access to resourses with in the neighborhood they have a recreation center for the seniors that provide them with breakfast and lunch and different array of services such as health care transportation services recreational activities and counceling given by the JCC (jewish community center) through the nyc department of aging. the new york department of aging mission is to work to eliminate ageism and ensure the dignity and quality of life of nyc’s older adultssand give support to the cargivers of these aged adults through service advocacy and education.Dfta.gov).
2. The available programs and services for individuals aged 65 and above in my community of Norwalk, Connecticut, and examine whether there are any gaps in the current system. As we are living longer, it is crucial to assess whether the existing resources adequately meet the growing needs of this age group. Having an elderly parent with Parkinson’s Disease I know allot of these services and my father and family have both benefited from them personally. In Norwalk, there are several local, state, and federal programs and services that cater to the needs of older adults such as Senior Centers, Healthcare Assistance, Transportation services, Home Care and Support, and Legal Aid and Counseling.
Norwalk is home to multiple senior centers that offer a wide range of activities, socialization opportunities, educational programs, and health services tailored to older adults. Connecticut’s Medicaid program, known as HUSKY Health, provides essential medical services and coverage for eligible seniors. Additionally, Medicare offers health insurance options for individuals aged 65 and above. ” Medicare helps older people pay the high cost of health care. It has two parts: hospital insurance (Part A) and supplementary medical insurance (Part B). Everyone 65 or older who is entitled to monthly benefits under the OASDHI program gets Part A automatically, without paying a monthly premium. Nearly everyone in the United States 65 or older is eligible for Part B; Part B is voluntary, and beneficiaries are charged a monthly premium. Persons with a disability under age 65 who have been getting Social Security benefits for 24 consecutive months are also eligible for both Part A and Part B, effective in the 25th month of disability.”
Norwalk Transit District operates the “Norwalk Wheels2U” service, providing door-to-door transportation for older adults who may have difficulty using traditional public transportation. Various home care agencies in Norwalk offer assistance with daily living activities, personal care, meal preparation, and companionship for older adults who prefer to age in place. Organizations such as the Southwestern Connecticut Agency on Aging (SWCAA) provide legal assistance, counseling, and advocacy services to address the unique challenges faced by seniors.
While Norwalk has implemented several beneficial programs and services, I have found that there may still be some gaps in the support system. Some potential areas of improvement include availability of affordable and safe accessible housing options specifically designed for older adults is an ongoing concern. Enhancing access to mental health resources tailored to the needs of older adults is so essential. This includes increasing the availability of geriatric psychiatrists and counselors who specialize in addressing the mental health challenges faced by seniors. As well, providing comprehensive support services for family caregivers, including respite care, training, and financial assistance, can help alleviate the physical and emotional strain they often experience.
It is important to evaluate whether the current resources are sufficient to meet the needs of this age group by considering the increasing life expectancy and the growing population of older adults, It is possible that the existing array of services may fall short in some areas, requiring additional funding, infrastructure, and community involvement to bridge the gaps effectively.
Question: How can we enhance the support for our senior population in light of the changing culutral demographics and evolving needs?
1. Burnout and compassion fatigue are common concerns in the helping profession
1. Burnout and compassion fatigue are common concerns in the helping professions that can be tied to several factors. Consider working in this field—including past or present experiences—and discuss the environmental issues that can cause burnout or compassion fatigue (i.e., managerial style, culture/climate, physical environment, client issues, colleagues, etc.). After explaining the challenges of 2-3 factors, suggest improving those factors in the organizational climate and ultimately reducing the likelihood of burnout or compassion fatigue.
One thing that can be discouraging is organization and communication in the business.
Another thing that can be tough in a work environment is feeling disrespected, whether about work or just being treated poorly by coworkers.
Another topic that can be difficult is the caseload and performing as expected. At the same time, there may be others that need to meet the expectations and needs of clients. Witnessing that can be exhausting because you know that more can be done.
Ways to improve these issues can be discussed in weekly meetings to discuss and communicate how the organization can be run better where the needs and concerns of everyone are taken into consideration, and everyone is meeting those work expectations. There are other ways that burnout can happen. However, if the organization’s flow runs smoothly and there is support, the chances are less likely because being understood makes a tremendous difference. When there is one factor rather than five, it makes the work more manageable, but when there are five factors, there is likely a high chance of fatigue and burnout.
2. Many people who work in this field experience burnout and fatigue due to the fact that they work many hours and even sometimes have to work overtime because of the need. Depending on the area you live in will have an impact on the number of hours and time you have to put into your job because the amount of clients you have could be more or less depending on this. Here in El Paso, their are many immigrants and homeless. The need for nonprofit organizations are high so their for the amount of burnout and fatigue is high on these workers. The lack of people in these kinds of jobs make the work load overwhelming to the ones who are doing this job.
