United Airlines Flight 232 was a regularly scheduled route from Stapleton International Airport in Denver, Colorado, to O’Hare International Airport in Chicago, Illinois.
On July 19, 1989, at 3:16 p.m., this DC-10 (registered as N1819U) experienced a catastrophic failure during cruise flight of the No. 2 tail-mounted engine due to an unnoticed manufacturing defect in the engine’s fan disk. The separation, fragmentation, and forceful discharge of the stage 1 fan rotor assembly parts from the No. 2 engine led to the loss of the three hydraulic systems that powered the airplane’s flight controls. The flight crash-landed at Sioux City, Iowa.
Of the 296 passengers and crew on board, 112 died during the accident while 184 people survived. Despite the deaths, the accident is considered a prime example of successful crew resource management because of the large number of survivors and the manner in which the flight crew handled the emergency landing of the airplane without conventional controls. Even though deemed successful, it is also the deadliest accident in the history of United Airlines.
