MOCK INFO: M is a 20 year-old, Hispanic female. She is currently presenting with a mental health provider for ADHD. She also mentions she has recently had symptoms of insomnia. She mentioned she has a lot on her mind is struggling with her relationship with her boyfriend. She has had some issues with finding the medication due to an Adderall shortage, and when she doesn’t have her medication, she cannot focus in college and then it causes her to have a bit of anxiety as well recently. She recently has felt depressed. It comes and goes, and she would to be connected to a therapist. Patient denies any past medical history however, she has a history of childhood asthma that she has outgrown and she’s very active with hiking. Current meds Adderall, 10 mg once a day MAIN/Working diagnosis: F90.9 Attention-deficit hyperactivity disorder, unspecified type DifferentiAL DIAGNOSIS : F31.9 Bipolar disorder, unspecified No known allergies Miss M currently lives with two roommates. Both parents are living father suffers from bipolar. Has two older brothers that she’s very close with. Is studying criminal justice second year of college mention she’s struggling with. Finances and she hates to ask her parents for money. Family hi: miss D mentioned her father was diagnosed as bipolar and is still married to her mother. Miss M mentioned her mother is closer to her and has been a great support when she was dealing with her childhood asthma. She had many trips to the hospital, and her mother she recalls was the only one ever with her there. Her two older brothers are married and live out of the house, but she speaks with them on a regular basis.
