PURPOSE: To recognize unusual courage or competence in an emergency, either on or off duty, but
beyond the call of duty that warrants special recognition
JUSTIFICATION: The justification for the award will be typed in Microsoft Word, in bullet format,
and should not exceed one page.
CITATION: The citation must be written in third person, include the job designation or organization,
period for which recommended and statement of achievement. Format must be justified, 90 words
max, and contain opening and closing line.
