Please use the attached slide deck, which is my Quality Field Project. It involves steering patients to IVX infusion centers and away from health systems — which we are going to institute a policy change from the top down in Illinois for Broad PPO Health Plan Members that these infusions (the ones in the slide deck) must be done at an outpatient infusion center. That will be the quality field project “intervention.” The policy will go in effect on January 1, 2024. Following that, we will track data to see if members/patients are in fact going to these infusion centers vs the health systems and the cost savings. The data points to include for JMP software are time to administration of drugs (from time of order– is this faster and more convenient for patients than going to the health system?), adherence to infusion regimen, and cost savings. You will have to create the data entirely (use your best judgement for pre and post-intervention data points and JMP software for analysis), based off of the slide deck I provided you. Please create the driver and fishbone diagrams etc.. Also please create physician (oncologists) surveys and responses and patients surveys and responses.

“III. Field Project Status Report #1
By about the beginning of November, you will have completed the planning and preparation for your Field Project, and initiated project implementation activities.
The report will include the following sections:
Executive Summary of Project Status: Here, briefly summarize whether your project is on track according to your Field Project Outline/Design submitted in September, what the key outstanding project issues/concerns are and whether you made any changes to your project since September, and what, if any, risks you see to the timely progress and completion of your project. Note that there is no need to repeat information unchanged from your Project Outline/Design report, such as the introduction, background/rationale, team, etc., as we can refer to your September report for that information.
High-Level Project Plan Update (key activities completed, upcoming, status of milestones, timeline): here, you can paste your high-level project plan spreadsheet (with appropriate highlights, such as any activity changes or delays) and then also expound on any issues you concisely highlighted in the Executive Summary and Project Plan, above, such as any specific problems affecting your scope and timeline.
Data/Analytics Update: In this section, specifically elaborate on the status of your data— and analytics—related activities, and any pertinent questions or issues you may have. Copy and paste your updated process maps, “fishbone” diagrams, driver diagrams, Pareto charts, histograms, control charts of your baseline project data, … etc. Additionally, upload an Excel or JMP file of your data, alongside your file (or explain why you are unable to do so).
Lessons Learned: Here, describe what you learned about effectively conducting a QI field project, what you should have done differently/better, … etc.
This document should be a single MS Word document, maximum of 5 pages in length, double-spaced, 1” margins, 12 pt. font—including any exhibits. I.e., do not submit separate Word or Excel files (e.g., of your project charter, project plan, etc.) as exhibits or attachments alongside your report. Rather, paste those directly into the body of the report. The one exception to this is the separate Excel or JMP upload of your collected data, as noted above.
Additionally, submit a set of up to 10 PowerPoint slides summarizing your key field project status information. Note that you can also choose to, instead of submitting both the Word and PowerPoint documents, submit only a PowerPoint document (with additional, brief information you deem necessary in the Notes section of slides).”
