Provide a questionnaire for your project. It can just be in a word doc or a copy from some survey software. Below are guidelines from chapter 11.
Put into survey monkey or some other survey software that can give you data in excel or allow you to do charts directly from it
DO NOT CONDUCT THE SURVEY UNTIL PROFESSOR HAS APPROVED QUESTIONNAIRE. You will have chance to revise and resubmit in a separate dropbox file

What questions will best answer the research objectives?
How should questions be phrased? Closed and /or open ended
In what sequence should the questions be arranged to encourage unbiased responses?
How do we encourage complete responses?
How should the questionnaire be pretested and then revised?
How long should it be?
See the questionnaire below- this is before the questionnaire was programmed into the survey software to account for what ifs or go to this questions if answered etc etc. Make sure you look at the RFP an examples below. I also want you to use these questions. Maake sure you also provide the survey monkey link so we can edit an put more questions .
