Compose a 525-word paper that establishes the current existing situation by completing the following:
Identify who comprises your patient population and describe your practicum setting.
Include key demographics including, age, income, race, education, housing, geographic area, etc.
Key cultural and/or lifestyle aspects including living conditions, beliefs, religion, values, attitudes, lifestyles, etc.
Key health identifiers including genetic health trends, tendencies toward obesity, etc.
Describe the disease process that you have selected based on the Healthy People 2030 objectives.
Using the advanced health assessment data from your clinical activity, summarize the findings of the advanced physical assessments you performed in your direct clinical activity.
Your summary must include comprehensive medical (past medical and surgical history, current medications, any allergies, etc.), social, and behavioral histories.
Analyze the patient population’s current condition by applying advanced pharmacology and advanced pathophysiology knowledge obtained in this program and from the evidence-based research.
Select 1 leadership and 1 nursing theory used by the advanced practice nurse that aligns with the project topic and patient population. Examples of theories: Orem’s selfcare theory, servant leadership theory, health belief model, caring leadership, Pender’s health promotion model, etc. Briefly explain how they align to your project.
Based on your assessments and application of advanced leadership and nursing theories, identify achievable health goal(s) or health outcomes for your patient population.

Task 2 – Needs Assessment
The purpose of the needs assessment is to identify and understand the safety and advocacy needs of the chosen patient population. Refer to the Current Situation Assessment worksheet and the answers you provided regarding the current outcomes from the current patient education practices, the desired outcomes, and the differences you identified between the two.
Identify the learning needs of the patient population by considering the following questions:
What is the “desired learning outcome we want to achieve,” and why? “What is the problem or opportunity to be addressed?” (Dickerson, 2014)
For the intended patient population, what is the potential difference “between where they are now and where they need to be” to achieve the outcome? (Dickerson, 2014)
What is the specific need for the patient population to eliminate the difference? What evidence “demonstrates the knowledge, skill, or ability needed” to eliminate the difference? (Dickerson, 2014)
How might you apply advanced practice nursing knowledge to educate the patient population to address their issue or problem?
Write a minimum of 175 words in a detailed summary of this identified need that is not being addressed by the current patient education standards.
Task 3 – Conduct a Synthesis of Current Literature
Research current scholarly information on the identified disease process for current best practices for prevention, management, and treatment.
Research best practices related to your patient population identifying commonalities and barriers to learning, disease and lifestyle management and prevention.
Examples of information that you will want to extract from your literature review are:
Evidence-based best practices
Best practices for managing and or prevention of the disease process
Best practices for educational interventions
Review 10 academic, peer-reviewed sources. Sources should be no more than 5 years old.
Create a 1,750-word (5-page) literature synthesis. Include a discussion of the following:
Management and Prevention – Evidence-based research found on current best practices for disease prevention and management to improve patient outcomes.
Education – Research found on best practices for educational interventions that will provide the best learning outcomes for your patient population.
Patient Population – Research found for best practice interventions for educational and disease management with consideration of the social determinants of health identified for your patient population.
Include a reference page with all 10 sources (the reference page does not count as one of the required 5 pages). See the Literature Review Example.
