Details This is an APA formatted individual paper performing a community assessment of your chosen population. This paper will address the behavioral risk factors and SDOH of your community in regard to the health concern of choice. The health concern of choice is hypertension and heart disease and the community is African Americans in the Hartford, CT area. I have attached the grading rubric as well as the sites needed to get data from. This is a series of papers so I have also attached the first paper of this series.
