1. The First Why will be – “The vehicle will not start because the battery is dead.”
2. The Second Why will be – “The battery is dead because the alternator is not functioning.
3. The Third Why will be – “The alternator is not functioning, because the alternator belt has broken.
4. The Fourth Why will be – “The alternator belt was broken, because the alternator belt was well beyond its useful service life and not replaced.
5. The Fifth (Root Cause of the reason why the vehicle will not start) :- Why will be – “The alternator belt was beyond its useful services life and not replaced because the vehicle was not maintained according to the recommended service schedule.
Problem 2 – We didn’t send the newsletter for the latest software updates on
time.
1. Why didn’t we send the newsletter on time? Updates were not implemented until the deadline.
2. Why were the updates not implemented on time? Because the developers were still working on the new features.
3. Why were the developers still working on the new features? One of the new developers didn’t know the procedures.
4. Why was the new developer unfamiliar with all procedures? He was not trained properly.
5. Why was he not trained properly? Because the CTO believes that new employees don’t need thorough training and they should learn while working.
