1. Choose a patient for whom you have cared or a person from your family or church who takes
prescribed medications on a daily basis. Select this patient wisely; not too complicated and not
too simple; enough of a case to address the criteria selected below.
2. Give a short review of the medical history. Obtain data from chart review (if available) and
patient interview.
3. List the medications the patient is on. Name, class, mechanism of action, and reason why
THIS particular patient is taking the medication. List the times of day the patient is taking
his/her medications and how many pills make up the dose. This is to be done in table form (form
provided at the end of this assignment description).
4. Identify any non-prescription medications the patient is taking and discuss how this affect the
prescribed medications –don’t forget herbals and vitamins as well as OTC medications.
5. Identify any special teaching that should be required, being sure to include timing of daily
regimen to minimize drug-drug or food-drug interactions.
6. Analyze the regimen for complexity and then make a prediction about how this patient will be
able to adhere to his/her medication regimen.
7. Identify any issues that are specific to the patient and the medication administration. This can
be things like refusals or reluctance to take pills, trouble swallowing, not understanding, cultural
or religious barriers, etc. How can you help the patient overcome these issues?
