(a) What have been the highlights of the individual’s life? When did augmentative communication become a part of his or her life, and how has it contributed to the individual’s accomplishments?
(b) Describe the augmentative system used by the person. For aided systems indicate the brand name of the equipment and the means of indication used. What symbol system is used? Describe the layout of the overlay(s). Describe how the student is positioned to use the system.
(c) Describe the person’s vocabulary on the augmentative communication device. Include specific examples of vocabulary use that you observed and other vocabulary the person is reported to use.
(d) How effective were the user’s attempts at communicating with the system? What problem areas can you identify? What factors, such as the vocabulary available or the behavior of the communication partner, influenced the success of the communicative exchanges?
