Client was visited today via telehealth. Client was in the community during interaction. Remained alert and oriented x4, verbally articulate, no skin change, and maintained stability till time of visit. Client verbalized she showered when opportunity to do comes. Some times he uses water to wash head, part dry his body very early in the morning. Client denied ability to follow routines and set daily plans. He does whatever comes on his way and never set daily plans. Client was encouraged to set daily goals and ensure it is achieved. Daily plans should be set based on priority or order of importance. Follow the rubric below. Assigned nurse and CSWS will work with client to set planning according to priority.
