Criteria
Letter Formatting
The letter includes a salutation, body, acknowledgement statement, and signature and date lines.
Body
Paragraphs 1 – 2: Briefly explains the issues leading to the creation of the group and research to provide support for the group or to better explain the issue.
Paragraph 3: Details about the group such as the length of the group (e.g. 10 weeks), how many people in the group (e.g., a maximum of 5 group members) and topics for sessions (briefly)
Paragraph 4: Address confidentiality and its limitations
Paragraph 5: how to reach you should there are questions or concerns.
ACKNOWLEDGEMENT STATEMENT & SIGNATURE LINES
Paragraph 6: The letter has an acknowledgement statement which includes the phrase “by signing this form, you acknowledge that you have read and understand the above information and agree to participate in this group (put the name of the group).
Signature and date lines provided for the participant and the group leader.
