HERE IS AN EXAMPLE:
Substance abuse can be a very challenging field for a new practitioner. It was generally thought that substance abuse disorders and mental health disorders were two separate issues (Halter, 2018). It is now understood that substance use disorder is categorized with the more traditional psychiatric illnesses.Substance use disorders often involve these following psychoactive substances: alcohol, caffeine, cannabis, hallucinogens, inhalants, stimulants, tobacco, opioids, sedatives, hypnotics, and antianxiety medications (American Psychiatric Association [APA], 2022). Substance abuse has become prevalent in many communities across America and has affected patients from every socioeconomic status. Patients who suffer with substance use disorders have greater risks of complications or death (Knopf, 2023; Ware et al., 2023). Within this first week of precepting, there was a patient that struggled with their goal of recovery of opioid use disorder.
This patient had been taking buprenorphine for a period of a few months and has avoided acquaintances that brought her back to the low points of her addiction. This patient had developed improper coping mechanisms especially once her opioid use disorder was being addressed. The patient resorted to a new experience of heroin and psychoactive mushrooms. During her regular visit, she addressed the behaviors that led to the drug abuse and interventions were identified to help this patient to avoid this sort of relapses in the future. Although there was more education and therapeutic communication to be had, the patient needed to run back to work to support her family.
Buprenorphine is often given as a medication to help patients with opioid use disorder. This medication helps reduce cravings and diminishes the pleasure of opioid consumption (Stahl, 2021). Although it is important to implement cognitive behavioral therapy techniques to influence this patient’s care, this can be challenging to an individual who does not take the time to work on some of their behaviors. As a new provider, it is important to realize one’s limits and make referrals when needed for more structured treatment to ensure patient safety and satisfaction (Knopf, 2023). Within the facility, there is a focus on insight-oriented therapy. This is an umbrella term that includes techniques from cognitive behavioral therapy, gestalt therapy, humanistic psychotherapy, psychoanalytic therapy and psychodynamic therapy (Halter, 2018). Substance abuse is often complicated with complex behavioral traits that this form of therapy can alleviate.
Increasing the patient’s self-esteem would help the patient take and identify responsibility for issues in their life (Wheeler, 2022). Creating better coping mechanisms can give patients strategies to navigate future issues with ease. One of the most challenging topics to educate the patient on would be to be self-aware. Being able to reflect on one’s history can help patients identify possible triggers to avoid relapses with substance use (Ware et al., 2023). This patient would be a challenging case for a new provider, but allows for a few learning opportunities that will be essential in future practice.
