The CEO of your organization has decided to evaluate the occurrence of hospital-acquired conditions (HACs) in your facility compared to other facilities in the area. You are tasked with doing an investigation that involves reviewing the data for the HACs. To get started, you investigate the types of HACs found in your organization. Then you develop a plan to gather the supporting information you will need to report back to the CEO. You realize you will need to collaborate with different people in your organization, such as the clinical leadership team and the information technology team. Some things you need to consider include the types of data associated with the HAC, which can be gathered from various data sources, and the best ways to gather and extract the data you will need.
Directions
Using the Module Two Case Study Analysis Template in the What to Submit section, complete the case study analysis. First, choose one hospital-acquired condition (HAC) to be connected to data components. You must identify the HAC in the introduction paragraph of your case study analysis. Then, connect data elements to the chosen HAC. Use at least three scholarly, current (published within the last five years) resources to provide evidence-based support for your work.
Specifically, you must address the following rubric criteria:
1. Hospital-Acquired Condition: Identify a hospital-acquired condition.
2. Types of Data: Describe at least two types of data that can be gathered from data sources associated with the HAC. Include the following in your response:
A. Data that can be gathered from the EHR
B. Data that can be gathered from other sources within the organization
C. Data from other organizations that can be used for comparison
3. How the Data Can Be Gathered: Describe how the data can be gathered. Include the following in your response:
A. What software is available to gather data?
B. What other data collections tools may be needed?
C. How can the data be extracted from sources such as the EHR?
