● Identify the practice issue or topic, and explain how it is relevant to your work as a social worker with this
case.
● Identify appropriate screening, assessment, treatment and/or intervention approaches, and
evaluation strategies using class materials and other resources that you deem relevant;
identify specific tools and techniques you will use.
● Identify ethical challenges and how you have applied a social and racial justice lens to
your work.
● Explain your choice of approaches, context of practice, tools, techniques, and strategies.
● Develop a clear intervention plan.
● Your paper must substantively apply and integrate course and other sources in an
adaptation of AMA style (meaning numbers corresponding to a reference page rather
than authors’ names and year that is used in APA).
● What other professionals should be included in your planning (a nurse to help with
patient education or an OT for a functional assessment or a community health worker for
connection to resources, for example)?
● Please refer to the rubric for more information! Be sure to include citations using AMA.
