Week 5 Project Community Health Assessment: Research assessments from any community, include your city/county. Don’t “create” data but draw from other resources as needed. The point is to consider the assessment data. Develop a statement of the issue based on the assessment(s). Quantify the issue, considering both health impact and economic impact. Research the scientific evidence. What studies are there? What other interventions have worked? Develop and prioritize interventions. Develop an action plan to include implementation of the interventions. Discuss how you may evaluate the program or policy. Submission Details: Write a 10-12 page, double-spaced paper in Word format. Apply APA standards to citation of sources.
