Chapter 1 Introduction to Health Assessment and Social Determinants of Health.
Physical Examination and History Taking is relevant to the diverse healthcare arena. Nurses are at the front lines, coordinating and providing holistic patient care in many healthcare delivery systems. The ever-changing world and complex patient needs to allow nursing to coordinate and provide holistic care for patients in many venues.
Nurses oversee the holistic care of patients. The nurse’s initial role in health assessment is to collect accurate health data.
Below is a brief nursing encounter with a newly admitted patient:
Mrs. J is admitted with a change in mental status. Her family has noticed many changes in her behavior this past week. She was found wandering around her neighborhood without any shoes on. When her daughter called on the phone, Mrs. J kept asking, “Who is this?” She is unable to state the date or name of the current president. She is unsteady on her feet and needs assistance getting to the bathroom. She is having some difficulty feeding herself (which the family is unaware of being a problem).

-With those contributing factors in mind, how could Josie’s death have been prevented, and what process changes would you recommend to avoid a similar tragedy?
-How could the hospital and its providers have given Sorrel King more power over how Josie was treated?
-Put yourself in the shoes of one of the clinicians who cared for Josie. How would you have reacted when Sorrel said, “You did this to her, and now you must fix her”?
-What can you do to make patients feel respected and included in decisions about their health care?
