My professor is requesting that we construct an intake report based on a case study that she provided to us.
Below are the paper details.
Read the attached case study and use the information provided to produce a narrative report with information in the following sections:
Presenting Problem
Background Information
Family Medical/Mental Health History
Developmental/Social History
Academic Information
Assessment of Present Functioning
Diagnostic Impression (DSM 5 diagnosis with mapping)
Treatment Recommendations
You may find that you have only minimal information for some of these sections; however, include everything that is relevant.
When you are completing the mapping section, please write out what each criteria state. Be sure to include the ICD-10 CM code or any of the required specifiers.
