Based on the problem identified in your Community create a Health Promotion Activity. It’s time to promote the community’s health by formulating a Health Plan.
Objectives:
Communicate effectively with adults by adapting language, grammar, and teaching concepts with the appropriate cognitive development.
Collaborate with a community health care provider in promoting positive healthy behavior.
Address learning needs of an adult population based upon identified health risks and health behavior.
Implement a health promotion program that is age-appropriate.
Select a method of evaluation which supports the health promotion program.
Instructions:
1. Select a health concern or problem identified by the community during your assessment. Identify three learning objective for health promotion activity.
2. Develop a health promotion program which addresses a health related problem or a topic of concern supported in above. Where will this program be offered/located for the community to access feasibly? Will it be specific to an age group or population? How will this program be funded if approved by community? Is it cost effective?
3. Select a method to evaluate learning or change in behavior for the topic of the identified population. Was it effective? How long will it take to meet the learning or change in behavior?
4. Provide (2) Literature Reviews related to your Health Promotion Program.

Submission:
Professional nurse will submit a brief health promotion activity and teaching plan.
Submit a minimum of 750 count words using Word document (.doxc) using Turnitin by Sunday, 23:59.
Use APA formatting and provide (5) references relevant to your topic. Title page and Reference page don’t count towards the 500 words. No abstract form is needed. No late submissions will be allowed.
