Refer back to the Stress Diary that you created for yourself. Select one of the sources of unpleasant stress that you identified*. Comment on your stress by stating:

Level of severity
Potential consequences if left untreated
Using that stressor as a prompt for yourself, create a self-care plan. You can use the Self-Care Plan Template as a guide, or come up with your own format that is appropriate for your stress. If you choose to use the self-care plan as your guide, please alter it as you see fit, deleting and adding sections. You will want to re-create the content so it is personal to you and relevant to your stressor.

Four strategies on how you will take care of your body and mind. Include reasoning for these strategies and evidence of their effectiveness
Four strategies on how you will deal with your emotions. Include reasoning for these strategies and evidence of their effectiveness.
*if you did not identify particular stress during this period, you can select something more general and ongoing that you deal with. Provide a rationale for not selecting something from your diary and state why your selected stressor is more pertinent.
