attend: All health care professionals and front line staff who work in the health care
industry. No form of taping is allowed nor are laptops permitted in the seminar, unless
used for the sole purpose of viewing the digital handbook. Registration form: Person’s

permitted in class. The seminar is approved for 7 CEUS for Nurses with the CEU
Network in most states for an additional fee of $28.00 and information will be provided
to you in class.

AT NO TIME CAN YOU ADVERTISE THAT YOU ARE TEACHING A CERTIFIED
DEMENTIA PRACTITIONER SEMINAR.
Certificate of Attendance / Homework:
The certificate must have the seminar name “Alzheimer’s Disease and Dementia Care
Seminar”, certificate of completion is presented to the person’s name, NCCDP Instructor
name, signature, and Instructor number (for now put NCCDP 00000), full address of the
location of the seminar, dates of the seminar, time of seminar (8:30 A.M. to 5:00 P.M. if
taught in 8 hours.) We recommend 12 hours when presenting to employees at Nursing
Homes, Assisted Living, Adult Day Care, home care, hospice and hospitals. You will
also add the CEU information if you are providing CEUs and the student’s name.
Only the individual who is attending the CADDCT ® seminar will be awarded the
NCCDP instructor number authorizing them as Certified CADDCT NCCDP Alzheimer’s
Disease and Dementia Care Instructor. Only the individual who attends the seminar is
provided with the materials and ownership of the materials. These materials can only be
taught as “live instruction”.
