Mr. Cortez assessment was completed today via telehealth. Client was alert and oriented, stable, verbally compliant, reported intact skin, and denied pain. Client recently started living alone after occurrence of stabbing. He reported to be working in a Universal cleaning company where he works 40hrs a week. Client recently went to hospital because he was stabbed. He reported to be having occasional pain on the site. Stitching was completed on the stabbing site and client verbalized site was not fully intact. Nursing education on skin integrity was completed.
