TITLE OF MY PROJECT: Development and Implementation of an Antidepressant Medication Adherence Protocol in a Community Outpatient Health Clinic; A Quality Improvement Project.
AIMS OF THE PROJECT:
Improve patient medication adherence
Decrease the severity of depression as measured by PHQ-9 scores
Improve patient knowledge related to antidepressants and depression management
Gain better insight into the adherence barriers of patients who seek care at this clinic
PROJECT METHODOLOGY:
Plan
We will develop the protocol and a new workflow.
We will Educate the team.
We will develop data collection tools.
We will Create/obtain educational materials.
Do
We will utilize PHQ-9 and MARS-10 screening tools
We will collect demographics.
The patient will Provide feedback on survey results and education, Encourage medication compliance and disease self-management.
Have patients download the medication reminder app.
Arrange follow-up visits and set an EMR trigger for rescreening during the appointments
Study
Analyze data: run chart, pre/post-survey scores, t-test results, demographic data, workflow efficacy, app use.
Act
Adopt, adapt, or abandon protocol.

5. Draft a chart with the sections below (word document or Excel spreadsheet).
6. Find the link “for Authors” or Authors information”
7. Add the following information from at least 3 different journals:

About: What is the aim and scope of the journal?
Name of Publisher
Types of papers accepted
Language
Disclosures (required)
Authorship directions (some journals may not have specific details in this section)
Directions for submission of manuscriipts
Specifics for Manuscriipt preparation: (abstract, title, word count/limit, ethical considerations, APA/AMA etc, directions for letters to the editor, keywords, title page, and peer review) Note, these should be added to your chart as a “summary” no need to copy and paste nor add the exact content from the website.
Add other details that your DNP Faculty Lead suggests that you can add to the chart.
