DIRECTIONS AND ASSIGNMENT CRITERIA
1.Complete the required
number of active learning templates (ALT) based on performance of the
practice CMS Assessment taken in Week 5.
2.Topics selected for the
ALT completion is based on the identified areas for focused review on
the Individual Performance Profile.
a.
In this example, the ALT focus is the Management of Care category and
Assignment, Delegation, and Supervision subcategory. The specific topic
for review is Delegation and Supervision: Delegating Tasks to an
Assistive Personnel and the Basic Concept ALT should be used.
b.To remediate the practice CMS Assessment taken in Week 5, use the percentage scored and these guidelines:
i.92% and above = at least 3 ALTs
ii.84-91% = at least 4 ALTs
iii.76-83% = at least 6 ALTs
iv.Less than 76% = at least 8 ATLs
v.NOTE: Faculty may require additional ALTs as part of the relearning plan.
3.These expectations apply to each ALT.
a.All
appropriate text boxes within the active learning templates are
complete; content is directly relevant to areas for focused review as
identified by the correlating Individual Performance Profile.
b.Handwritten critical points for each content or patient need area are included.
c.The ALT documents are legible.
d.The
ALTs tabs are organized in alphabetical order, except the Critical
Points Review document which is last in each section. ATLs are arranged
by date, with the most recent on top.
e.The binder is to be maintained in good condition. Binders not in good condition should be replaced.
4.A separate Critical Point Review document that includes three critical points for all topics missed on the assessment.
a.Record
three critical points for all topics missed on the assessment. The
topics missed are located in the Topics to Review section of the
assessment report.
b.In the example below, the topic missed from
the Topic to Review section on the assessment report is Legal
Responsibility: Disclosure of Inmate Health Status.
5.Loss of the binder will result in a zero for all course specific remediation points.
a.It is recommended that students scan and save their paperwork in case the binder is lost or damaged.
b.Faculty may require students submit scanned copies to the online course shell for grading and retention purposes.
6.For the students to achieve points:
a.All
criteria in the satisfactory column of the CMS Assessment Student
Success Binder Rubric must be met to earn the available ten (10) points
in each category.
b.Students can earn between 0 and 50 points based on points earned in each category.
