You are a paralegal at the law firm of Sky Sands and Watters (Virginia Beach Office). In an effort to
become more organized, create better attorney/client relations and possibly even avoid malpractice
suits and bar complaints, the senior partner has asked that you create a complete client checklist to be
used during the initial interview for the area in which you practice.

Client Intake (Checklist) Project Part One- The Plan
Review the lecture on Client Intake Forms and the associated links. Begin a Client Intake Plan. Answer
the following questions using full and complete sentences and correct spelling and grammar.
1. When will your client intake form be completed? Before or during the initial interview? Why?
2. What platform(s) will be used to complete the client intake form? Will you post the forms
online, through the firm’s client portal, via regular mail or at the meeting itself? Why did you
choose this (these) particular method(s)? What problems do you anticipate, if any, and how will
you resolve them?
3. Do you believe that charging for an initial client interview is a good idea? Why or why not? If
charging for the initial interview is not in your intake plan, will you limit the meeting time? Why
or why not? If you think you should charge, what is fair?
4. What methods will you invoke in order to ensure that your client is aware of the meeting time
and actually arrives on time? Do you think that a monetary penalty is in order if a client does
not make the meeting? Why or why not? What will your firm do to ensure that money is not
lost for a missed interview?
5. What type of law firm will you be drafting an Intake Form for (Practice Area)?
Client Intake (Checklist) Project Part Two- The Form
Review the lecture on Client Intake Forms. Choose one subject area that we have discussed in class (or
in another class you have taken) and complete the following assignment.
The first part of your intake form should be general questions. What information do you need from
every client regardless of the area of practice?
Add questions which are specific to the practice area that you have chosen. In addition to information,
what documents or other evidence will you require to assist your client with this issue?
You may use the form examples attached to the lecture or found on the internet AS A STARTING POINT!
These documents must be located by you (you are not to assist a classmate in any way or receive any
assistance from a classmate or other individual other than your instructor). Any forms used, regardless
of the source, must be adapted to this firm and this practice area. Be thorough. If done well, this can be
an addition to your interview portfolio.
