Chronic Pain Assessment
J.P. is a 15-year-old African American adolescent who was diagnosed with
sickle cell disease 13 years ago. J.P. presents to the emergency department
with pain related to sickle cell crisis that started at track practice about 2
hours ago.
Subjective Data
Pain level is an 8/10 location = bilateral legs, described as deep muscle
pain and upper left quadrant pain (visceral pain).
Student in 10th grade, honor roll student
On the track team
Lives with mother and father
Patient states pain gets better when he rests.
Walking and running make the pain worse.
Patient took NSAIDs at home before coming to the E.R. with only
minor relief.
Objective Data
Vital signs: T 37 P 101 R 20 BP 140/68
Weight: 140
Height: 5 feet, 6 inches
Patient is alert/oriented x4
Patient is grimacing and holding abdomen.
Abdomen is soft and tender in left upper quadrant
Legs show no edema, erythema or any signs of injury.
Patient’s gait is steady but slow.
