A nurse has an order to complete a procedure on a patient. After reading the procedure, the nurse notices that it is not best practice and it has not been updated for 6 years. The nurse takes this issue to the nurse leader (charge nurse) and the nurse manager (unit manager).

It is important to update policy and procedures as often as necessary in keeping up with the ever changes that are enacted by lawmakers and the changes that comes from the ever-growing health care and technology community. Policy and procedures should be reviewed annually to ensure they are consistent with the overall health care best practices. “At the heart of policies and procedures stand patients, and at the end of the day, the goal of policies and procedures is to protect patients from harm.” Rogers, Alison (March 12, 2017).
Utilizing outdated policy and procedures that are no longer being used or no longer recognized by the nursing organization as a whole can cause unnecessary harm to patient/s resulting in adverse consequences. As patient advocates, this is an ideal time to assume leadership positions to protect patient rights, ensure patient needs are met, and ensure the nursing profession retains its strength. As a nurse manager I could perform intermittent appraisals of the policies and procedures through a tracking system that will detect when policies and procedures are due for update.
