Since spirituality is a dimension of the human person that must be addressed for any true “healing” to be done (and which may, in fact, be a component of the original illness), it is necessary for health care professionals to determine their patient’s spiritual needs in addition to their physical and psychological needs.  One common way to do this is to administer an “assessment tool” which is a list of questions that will give the health care professional the necessary information.  Needless to say, this must be done sensitively and non-intrusively.  You will be reading an article about the design of an assessment tool and an example will be furnished for you.  It is best to administer such a tool once a relationship of trust and rapport has been established with the client.
You will prepare a list of 5 questions you would include in  your own assessment tool.  Only 2 of these may be questions published in an already established assessment tool.  (Include in-text citations and Works Cited for such questions.)  3 of the questions must be your own.  Try to be creative and design your questions in such a way that you would neither offend nor intrude upon your client’s innermost beliefs.  Your purpose is to provide for him/her some spiritual resources that will aid in healing.  You may consult additional published assessments if you wish. (Just be sure to cite.)  Conclude with a 3-5 sentence paragraph explain how your selection of questions would provide you with adequate information about the patient’s spiritual needs.
