How do you define quality and safety? What role does perspective play in your definitions?
From a healthcare perspective the Institute of Medicine (IMO) defines quality as “the degree to which health services for individuals and populations increase the likelihood of desired health outcomes and are consistent with current professional knowledge. They define safety in healthcare as “the prevention of harm to patients” (Wagner, 2021).
Perspective plays a huge role in the definition of quality and safety. Hospital administration might think quality is cost saving, while nurses think quality is fully staff units, an uninterrupted break to prevent burn out, and a patient with a positive outcome . The IMO released a report shining light on the issue of quality and safety in the healthcare system. The effort to improve quality and safety was focused on standardization with a variety of quality improvement tools and techniques recommended. Some of these tools included Total quality management (TQM), Lean, Six Sigma, Data analytics, and The Triple Aim. Despite improvements, these quality improvement tools were borrowed from the manufacturing industry, that is primarily applied to inanimate objects. While quality improvement tools such as Lean look amazing to hospital administration due to is success in cutting costs, The Joint Commission found Virginia Mason Medical Center (the first to implement it) was failing to meet multiple quality and safety standards. For example, hospital administration could implement a new quality improvement tool and find great success. They eliminate some positions, deciding to cut costs further, only to have staff working harder and longer hours. Resulting in increased burnout, unsafe conditions, and dissatisfied employees (Wagner, 2021).
Quality and safety have a different definition when it comes to the manufacturing industry and the healthcare industry. Humans vs. inanimate object. Patients experience and satisfaction are incredibly important for quality. The more positive a patients experience is, the more compliant they will be to their providers care plan. Quality improvement should focus on affecting people and their behavior. While you can’t measure peoples thoughts you can see peoples actions and compliance with their care (Wagner, 2021).
