According to Janet Urbanowicz (2019), registered nurses transitioning to the APRN role experience role ambiguity, intrinsic and extrinsic obstacles. As a new advanced practice registered nurse, the APRN may have thoughts of anxiety, role confusion, stress, insecurity, self-dout, apprehension, emtional turmoil and isolation. Provisions within the NP core competencies that foster role transition include; scientific foundation where the novice APRN develops new practice approaches based on information obtained via research, and preceptorship program. Practice inquiry by collaborating with collegues to apply clinical investigative skills to improve health outcomes. Utilizing health delivery systems and practicing independently thereby demonstrating the highest level of accountability for professional practice (DeNisco, 2021).

Transitioning to a new role has a different perspective even if you have years of experiences from an entry-level position. You must maintain an active RN license as an NP but practicing as a NP contrast to the years you’ve practiced as a nursing. The two NP core competencies that foster into the transition of roles are Quality Competencies (DiNisco, 2021). Employed as an RN, I’m constantly exposed to policy changes to improve quality care for patients due to near missed cases and sentinel events.
I’ve worked in four different hospitals throughout my nursing career and for each orientation I’ve attended, the educator mentions how the hospital is a dangerous facility. According to Patra, and Orlando De Jesus, the Joint Commission receives a report approximately 700 deaths per year for sentinel events. (Patra. P.K & Jesus. D.O, 2020). Each hospital I’ve worked for enforced policies and Root-Cause Analysis to prevent continuation of mortality inside of hospitals that’s caused by healthcare worker. Blood Transfusion compatibility is excessively enforced, and RCA is conducted to develop innovated methods to avoid incompatibility blood type to a patient. About 3 months ago, two RN were required to fill out a form to verify the patient. Ineligible handwritten, missing information from the blood compatibility document delays care and patient interventions. Currently, the policy has changed where two RN must scan the patient’s label within 5 minutes apart to be eligible for the blood bank to accept the type and screen specimen. Policies changes frequently to improve patient’s quality outcome. Trials are in process, RCA are utilized and transitioning to a NP role that’s focus on Quality competencies will facilitate with that role.
