Instructions
1. Please write an intro to the survey: you set up/explain the topic and tell the respondent why their answers matter. You also specify that all this is anonymous. Do not state the hypothesis as this might influence the respondent’s answers (sometimes respondents want to help!)
2. Write at least 15 questions and offer appropriate choices of answers (please see my silly sample)
3. Make sure that the 3 sections of the survey are clearly identified
