The patient from when I started nursing 12 years ago is an older, demented patient. For the most part all of my patients were bed bound and the few that were not needed at least 2 people for transfers. Many of these patients did not have family and were on their own, with no visitors. many of the patients were contracted and were older in age.the patients took all medications crushed and mixed in applesauce. Many of the patients did not have anyone to advocate for them. The patients were all total care and many of them were being tube feed by a permanent peg tube in their stomachs. The patients today are younger and sicker. There are more complex diseases and alot more complex management. The families sometimes make it very hard to care for the patient with dictating. I see a large increase in non compliance. When medical advice is given the patient may follow about 40% of the plan and if the plan fails then point the finger. The public health nurse would assess a larger amount of people. The goal is to educa‌‍‍‍‌‍‍‌‌‍‍‍‌‍‍‍‍‌‍‍te the larger amount of people. The health of the general public is the main priority. the community health nurse can also help patients with preventative care. The preventative care is also a general population problem therefore the public health nurse will have it on her radar. The pandemic, HIV and any other communicable disease the public health nurse can address. The clinical nurse has a much broader assessment in the medical world. The clinical health nurse usually does a focused assessment of the patient and will go back and do the other assessments later. The clinical nurse is evaluating the health of one person versus an entire population of people. The clinical nurse is the person that ensures that the patient does not die. Clinical nurses range from different position, even from the bedside, to educators and several other nurses. Clinical nurse can assess any body system and assist with improving it.
