Select an older patient with an acute condition(s) from your Nurse practitioner practicum experience this week.
Please pretend you gave her a physical and plan for treatment. Include related to a Chief Complaint (CC) on a patient. Pick acute otitis media for acute illness.
Then make up the following for that patient: ID that includes initials, age, DOB, gender, race, ethnicity, a chief complaint, Presenting Illness, Review Of the Systems, Vital signs, physical findings, and assessment with at least 3 differential diagnoses, a final diagnosis, and treatment plan in a full SOAP note format. Use a HEENT related Chief Complaint that a patient would present within an acute care setting.
Must include focused review of systems like cea
ICD 10 for assessenment (acute otitis media)must be included.
medication name dosage and all the details must be included under the plan.
follow ups too.
