C‌‍‍‍‌‍‍‌‌‍‍‍‌‍‍‍‍‌‍‍ase Study – Mental Illness and Drugs Overview: The client is a 48 year old female living in Los Angeles who has 22 months of sobriety from alcohol abuse. She is on disability for schizoaffective disorder and PTSD due to extensive abuse and neglect as a child. She sees her psychiatrist regularly and he has commented on her dependent personality traits which are characterized by her need to be taken care of and fear of being abandoned. You notice odd thought processes and odd comments. Such as: “she is missing her brain today – aliens’ took it last night”. Despite these oddities she keeps all her appointments with you, sees her psychiatrist and takes her medication regularly. She has not had any psychotic or manic symptoms since she has stopped drinking and has followed her treatment plan. She has a stable living situation and is capable of managing her monthly disability check. She pays her rent and other bills on time and has a live in attendant arranged through her case manager. In your last session she tells you she had to leave her apartment at 4 am this morning to find a shelter to stay in. When you ask why, she explains her attendant came into her room and violently shook her awak‌‍‍‍‌‍‍‌‌‍‍‍‌‍‍‍‍‌‍‍e demanding to know why she was spying on her. The attendant accused her of putting video cameras inside the walls of her room in order to watch her. Your client was frightened by the attendants threatening behavior and left her apartment to sleep in a homeless shelter for the night. The staff at the shelter didn’t know what to believe and called the police when she told them her story. However, she declined to file a complaint. She tells you she didn’t want to cause any trouble for her attendant. Filing a complaint might get her in trouble and cause her to lose her job! She tells you that her attendant has been very unreliable recently and acts very secretive. She’s also noticed money missing from her handbag the last few weeks. Answer the following questions, in detail: 1. What immediate clinical concerns does this client present to the counselor or therapist? 2. How does this client’s past and recent history of trauma contribute to her severe mental health problems? 3. Are there mandatory reporting requirements for clinicians to respond to and how would such requirements interface with confidentiality requirements? 4. Prioritize the clinical interventions that need to occur ‌‍‍‍‌‍‍‌‌‍‍‍‌‍‍‍‍‌‍‍with this client.
