In the first stage, you will perform an analysis of a chart that represents performance at a peer hospital. In the second, you will write a report based on your analysis that outlines your proposal to improve performance within your own hospital. This assignment supports unit learning outcomes 1, 2 and 3. Assignment 2: Clinical informatics analysis and report Assignment details Scenario You are the Director of the Acute Stroke Unit of a large hospital. Your CEO has been given a chart regarding the management of stroke patients at your hospital. The chart indicates that there may be issues with the quality of this management, and your CEO would like you to investigate reasons for possible poorer than expected performance. They have asked you to 1/ analyse the chart to identify any issues of performance and 2/ prepare a report that outlines your plan to address these performance issues within your own hospital. You will work through this assignment in two stages: Stage 1: Analysis (1600 words) Here is the chart provided to you: VLAD chart (2021) courtesy of Paula Bowman Before you can analyse this chart, you’ll need to understand how Variable life adjustment display (VLAD) indicators work. To do this, work through the materials on VLAD details—you’ll find a large version of the chart on this page too. Your interpretation of this chart will be guided by the following questions, and your answers to them will be included as an appendix to your Report proposal (see Stage 2): Question 1 (500 words) How are VLAD graphs interpreted? Here you will need to explain the important features/aspects of a VLAD chart. As an example: what does the red line, blue line, black line indicate; what are the numbers; what does the y-axis indicate; what are the level 1 to 3 limits for? Note that this is not an exhaustive list. There may be other features that you could explain (. what they are and their purpose). Question 2 (500 words) Describe the issues that are evident when you analyse this VLAD chart—what does the chart tell you? Question 3 (600 words total) Use the Pyramid of Investigation to describe: The possible underlying reasons for the change in patient outcomes over time—what could be the causes of the problems evident in this chart? (300 words) The actions that you would put in place to investigate each of the possible issues or problems that you have described in . (300 words) Stage 2: Report (2550–2850 words) From analysis of the VLAD data you have identified the following issues that have contributed to the recent poorer than expected outcomes for your patients: Casemix Increase in transfers of acute stroke patients from regional hospitals; poorer outcomes for delayed presentations to the ED of patients with possible acute stroke; poorer outcomes for Aboriginal and Torres Strait Islander patients. Resources High rates of outliers associated with unavailability of beds in the Acute Stroke Unit due to delays in discharge of patients. Process of care Long waits (> 48 hours) for CT scans on admission; delays in allied health assessment of patients admitted late in the week; inconsistent and incomplete documentation of physiological and neurological signs during the acute admission; failure to identify patients requiring palliative care and no palliative care pathway suitable for stroke patients in use. Based on your interpretation and analysis at Stage 1, develop a written proposal for improving the management of acute stroke patients at your hospital. The following information should be included in your proposal: Report structure Introduction (300–350 words) This section should contain: • A problem statement (150 words). • Definition of the scope of the problem to be addressed (150–200 words). Body (2250–2500 words / 30 marks) This section should include: • The desired changes or improvements and why these are required (750–1000 words). o This should be based on the scenario and background information provided and your own research about best practice. • The options for action you’ve identified (500 words). o What needs improving and what strategies could be used? • One priority action for implementation and a detailed plan for doing this using the Institute for Healthcare Improvement Model for Improvement/PDCA cycle (1000 words). o This is covered in PDCA cycle and quality improvement plans.
