After you finish making your appendix, write up your highlights and takeaways from using the tools in 3-5 pages of text (12-point font, Times New Roman).
Do not make a cover page and do add page numbers down at the bottom, centered

Any submission without an Appendix with each of the three tools detailed out will result in a failing grade.
