Ralph is a 16-year-old adolescent male who relocated from Puerto Rico to the United States two years ago. His mother and brother arrived with him for his intake at the local outpatient clinic counseling center. The family is Spanish speaking. Ralph is the youngest child of four children and the only one who resides with his mother and her on again, off again partner, with whom Ralph has a strained relationship. His mother was separated from his father when Ralph was 6 months of age due to a history of intimate partner violence. After his parent’s separation, he had little contact with his father and no formal visitations.
Ralph had been hospitalized for two weeks one year ago due to self-harm (cutting behavior) and suicidal ideation with a plan. He was diagnosed with depression but has had no treatment since his hospitalization. Ralph’s mother believes that he is impacted by his father’s rejection, and she sees signs that he is self-isolating. When asked, he is vague about his needs and expresses “humiliation” as a predominant emotion as it relates to repeating a grade two years ago upon entry to the United States and disrespecting his maternal uncle at a recent family event. His mother states that he is now doing well academically.
Ralph has significant relationships with his mother’s extended family who live nearby (uncle and cousins). His 20-year-old brother lives in Puerto Rico but was visiting with the family for a two-week period during the time of intake. They have a very close sibling relationship. His two other siblings are older and live in another state though he sees them a few times annually. Ralph’s mother describes the family as a strong unit.
She speculates that her son’s mental health symptoms are a symptom of rejection by his father. She thought it important for youth’s brother to offer his perspective and he shared a similar viewpoint. Specifically, Ralph is exhibiting behaviors of isolation and has increasing angry outbursts within the home. She is also concerned that he no longer plays the sport that he loves and has declined invitations to visit the uncle’s home since the recent incident of “disrespect” toward the uncle (despite the uncle’s insistence that he forgives Ralph). She does not want him to further deteriorate and require hospitalization. She also expressed suspicion that he is self-harming. Ralph simply thinks he is “humiliated” about the incident with the uncle and is vague about his overall mental well-being.
An intake and assessment must be completed for Ralph.
In your paper, address the following:
