A 42-year-old woman has noticed dry skin, fatigue, and weight gain over the past 3 months. Her blood pressure is 110/70 mm Hg, pulse 60/min, and heart and lungs are normal. Her skin feels rough and dry, but the rest of her examination is normal. Her biochemistry is normal, but the TSH is 39 mU/L.
Provide at least 3 possible differential diagnoses and justify your rationale. Develop therapeutic plan options based on quality, evidence-based clinical guidelines.

You have a 17-year-old female that comes in with suspected pregnancy and limited family support. She is accompanied by her 18-year-old boyfriend who is the father of the child.
While knowing she needs prenatal care, what resources and education would you try to provide and how would you encourage compliance in this vulnerable patient? What are some of the biggest risk factors that this patient faces and how can you provide quality care while being sensitive to the stigma that comes with teen pregnancy?

Vignette – You are seeing a patient 38-year-old female, you are aware that she has a significant problem with meth abuse that you have been working on. She also has a violent tendency. Today she comes in with her daughter that is 8 years old. She appears cold and lacks maternal boding. She yells at the daughter throughout your visit. The daughter spends most of the time crying. She grabs the child’s arm forcefully and then storms out. You did not see anything that the daughter was doing to elicit this kind of response from the patient. They leave and you are left with an uneasy feeling. What will you do
HPI: in your own words do not copy and paste.
Diagnosis: talk about the diagnosis you feel is presented with evidence on why you feel it is that dx.
Differential: differentials that may be pertinent (3), why they fit and how you ruled them out.
Treatment: be specific about what medication, dose and duration and evidence base on why the medication was chosen. Other non-pharmacological tx also presented.
Education: discuss education to patient, evidence based.

tympanic Membrane and Thyroid Gland
Using the classroom resources or South University Online Library, research the tympanic membrane and the thyroid gland. In a Microsoft Word document of 4-5 pages formatted in APA style, address each of the following criteria.
Two focused health assessment histories
One assessment related to the tympanic membrane and the other focused on the thyroid gland.
The assessments can be hypothetical patients or patients you have had in the past (remember HIPAA if you are describing a previous patient).

(a) [Repealed by Acts 2007]
(b) A nurse shall report to the Board in the manner prescribed under Subsection (d) if the nurse has
reasonable cause to suspect that:
(1) another nurse has engaged in conduct subject to reporting; or
(2) the ability of a nursing student to perform the services of the nursing profession would be, or would
reasonably be expected to be, impaired by chemical dependency.
(c) [Repealed by Acts 2007]
(d) A report by a nurse under Subsection (b) must:
(1) be written and signed; and
(2) include the identity of the nurse or student and any additional information required by the Board.
(e) Instead of reporting to the Board under Subsection (b), a nurse may make a report required under:
(1) Subsection (b)(1) to a nursing peer review committee under Chapter 303; or
(2) Subsection (b)(2) to the nursing educational program in which the student is enrolled.
(f) A person may not suspend or terminate the employment of, or otherwise discipline, discriminate against, or
retaliate against, a person who:
(1) reports in good faith under this section; or
(2) advises a nurse of the nurse’s rights and obligations under this section.
(g) A violation of Subsection (f) is subject to Section 301.413
