The client has two problems: 1) Depression and 2) Anxiety.

Question 6 (Total: 20 Points) Based on the case vignette, create a treatment plan for the client in the vignette. Use the example chart provided during week 5 for the format.
– Problems/Target of Treatment must match what is identified in case conceptualization (The client has 2 problems – Depression and Anxiety)
– 1 goal & 2 objectives must be identified for each problem/target of treatment
– A citation must be included if using a self-report measure
