This week, you should continue your Improvement Plan: Part 2. As you work on your Improvement Plan, be sure to incorporate peer-review feedback (as appropriate). This week, focus on the areas of:

  • Information technology (value-based care model)
  • Be equitable (IOM Aim)
  • Your country’s comparison to the value-based care model

To prepare for this Assignment:

  • Refer to your Community Profile and Continuum of Care document that you developed in Week 3 and return to your continuum of care scenario.
  • Return to your Improvement Plan: Part 2 document, which you should have downloaded in Week 7 and continued to update during the past 3 weeks.
  • Revisit the peer-review feedback that you received during the past 3 weeks and update your Improvement Plan based on the feedback you received.

Submit your Improvement Plan: Part 2 to address the following:

  • Summarize the original continuum of care plan submitted in Week 3, as well as the healthcare settings that you identified.
  • Propose at least one improvement for each of the following value-based care elements and IOM Aims.
  • Week 7: Systems integration
  • Week 7: Be efficient
  • Week 8: Geography of care
  • Week 8: Be timely
  • Week 9: Information technology
  • Week 9: Be equitable
    • Compare your community’s model of healthcare delivery to the delivery model of other countries that closely align with the following value-based care elements, including an explanation of what lessons—positive or negative—your community could learn from the other countries:
  • Week 7: Systems integration
  • Week 8: Geography of care
  • Week 9: Information technology
    • Provide a summary and conclusion for your paper.

Step 1: Summarize Original Continuum of Care Plan

  • Begin your paper with a summary of your Week 3 continuum of care plan

  • Include:

    • Healthcare settings identified (e.g., hospitals, outpatient clinics, community health centers, home care, rehabilitation services)

    • Key services offered at each level of care

    • How care transitions occur between settings

This ensures readers understand the baseline model before you propose improvements.


Step 2: Propose Improvements for Each Value-Based Care Element and IOM Aim

Your Improvement Plan should include specific, actionable proposals for each week’s focus areas.

Week 7:

  • Systems Integration: Suggest a method to integrate services across the continuum (e.g., shared electronic health records, care coordination teams)

  • Be Efficient (IOM Aim): Propose strategies to reduce duplication, waste, or unnecessary procedures

Week 8:

  • Be Equitable (IOM Aim): Recommend strategies to reduce health disparities across socioeconomic, racial, or geographic groups

  2. Expected outcomes (quality, access, equity, cost reduction)

  3. Feasibility (resources, stakeholders involved, technology required)


Step 3: Compare Your Community’s Model to Other Countries

  • Choose countries with strong value-based care models

  • For each element:

    • Systems Integration: Compare coordination methods (e.g., UK NHS integrated care networks)

    • Geography of Care: Compare rural/urban access strategies (e.g., Canada or Sweden telehealth networks)

    • Information Technology: Compare IT use for patient records and population health (e.g., Denmark or Estonia digital health platforms)

  • Highlight lessons your community could adopt, including:

    • Positive strategies to emulate

    • Potential pitfalls to avoid


Step 4: Provide a Summary and Conclusion

  • Summarize the key improvements proposed

  • Reinforce how your recommendations align with value-based care principles and IOM aims

  • Highlight the expected impact on patient outcomes, equity, and system efficiency

  • Optionally suggest next steps for implementation or evaluation


Step 5: Formatting and Submission Tips

  • Use APA 7th edition formatting

  • Include headings for each section: Summary, Improvements, International Comparison, Conclusion

  • Incorporate peer-reviewed references to support your proposals

  • Integrate peer-review feedback into each section where appropriate
