You are to complete an assessment of your patient/client’s stress level.
1. Have your client complete the following assessment tools that assess stress from the perspectives of the Environmental Model of stress and the Psychological Model of stress. (35 points):
The Social Readjustment Rating Scale looks for life events that may have occurred that can cause stress in a person’s life (Environmental Model).
SCORING
Each event should be considered if it has taken place in the last 12 months. Add values to the right of each item on the scale linked above to obtain the total score. Susceptibility to illness and mental health problems:
Score of 300+ = Major – 90% chance of getting sick within 2 years.
Score between 150-299 = Mild to Moderate – 50% chance of getting sick within 2 years.
Score below 150 = Low – 30% chance of getting sick within 2 year
List your client’s score and indicate which category he or she falls in.

The Perceived Stress Scale (PSS) examines if an individual perceives his or her life situation as stressful (Psychological Model).
Calculate your client’s score. The score is the total of the 10 items and should be a whole number between 0 and 40. See the directions on the form.
Refer to the Norm Table provided with the form to compare his or her score to the means (averages) provided based on his or her gender, age, and race. How does he or she compare to the mean (average) perceived stress levels of his or her peers based on gender, age, and race? For example, the Mean, or average, score for a male is 12.1, for a female 13.7. See the table for the means for age and race.
List your client’s score and indicate how he or she compares to the Mean (average) perceived stress levels of his or her peers based on gender, age, and race.
Do the Social Readjustment Scale and PSS seem to agree with each other, or do the results suggest different levels of stress?

2. Collect the following information (35 points):
What does your client do when they feel stressed?
Does your client do anything just for relaxation and/or recreation, or use any relaxation techniques?
Does your client have appropriate sleep patterns?
