Tri-fold Pamphlet: Create a TRI-FOLD pamphlet using the provided template only to educate clients on one of the following current patient safety issues: 1) Currently aging adult and how to care for his/herself at home 2) Medication-polypharmacy and how a patient cannot make a self-medication error 3) Fall prevention for the elderly at home **If you have another educational safety topic you would like to use, you must seek approval from your faculty. Be sure that your pamphlet addresses a safety issue at home. No pamphlets will be accepted if related to hospital safety. Your pamphlet must include the following items: Include at least 5 tips for preventive care for the patient. State why each are important. What is the rationale. Include images that match each educational tip. Be sure to state the importance of the education and tips. Include information that should be shared with family or caregivers and explain importance. Include local resources in your community that might be available for this type of safety concern. Note the resources are not your references. List address, number of resource. Include at least 3 current (published less than 5 years ago to date) references, in APA format. Refs need to be on the Tri-fold pamphlet, not on separate page. This must be a Tri-fold pamphlet. Please do not submit a handout or several pages of content. A tri-fold is one page that is double-sided when printed out. The front of the pamphlet must have the title, course, your name, school, date and faculty name. Do not use any infographic programs to create a pamphlet such as piktochart, canva, vinme, etc. Turnitin cannot capture the content as it views it all as an image.
