I have attached a screen shot example of what the code book should look, and then attached the code book I that started that you can utilize and plug in the remaining information in the excel cells based on survey. I have also attached a copy of the survey so that you can put the correct information for the code book. Then the code book has to be scrubbed with a write up of findings. This is for Master of Public Heath class this survey is based off of responses from 15 diverse individuals male and females different ages etc.
