1. When evaluating your experience at the agency, please summarize how your practicum experience met the requirements of the learning agreement. Give some examples that illustrate this. Also, summarize the areas that did not meet the learning agreement outcomes expectation.
2. Please discuss your view on how supervision for your internship was conducted. Include areas of strength demonstrated and areas of needed growth. Exemplify your statements.

Describe your access to and utilization of resources made available to you that enhanced your ability to grow as a practitioner within the agency (e.g. manuals, in-services, formal trainings, media and reading materials, staff development meetings, etc…). Discuss how these were helpful in shaping you as a practitioner.
